Q: How do I create an opt-in form for my website or a tab on my Facebook Page?
A: This depends on what you use to manage your list and email marketing. Whatever service you use whether Constant Contact, MailChimp, Infusionsoft (this is the platform I use) or another, they will have an area for you to create your opt-in form and then it will give you the code needed for adding that to your site or Facebook Tab.
I chose Infusionsoft because it allows me to do more than just email and I can keep track of where people sign up for my list by having a different form on each site. Plus, I have more flexibility when it comes to designing my opt-in form.
